Requesting Enrollment in HNRS 4000 or Documenting Enrollment in a Departmental Thesis Course

This form must be completed prior to the last day to drop classes of the semester of the requested course enrollment.

Note that only HNRS 4000 will be added to schedules as a result of completing this form. Those students documenting their enrollment in a department thesis course must request that course enrollment through that department. The purpose of listing departmental thesis courses here is to inform the Upper Division Honors Advisor of that course enrollment and to notify them of the commitment of your Thesis Director.
